Project Scope Summary (a general narrative of the needs and scope of the project) The Oklahoma Department of Human Services (OKDHS) seeks to engage the services of a vendor from the SW1025contract in the Attachment C, Programmer/Developer Analyst III to develop and maintain the requirements and functionalityof the KIDS system in accordance with the terms and conditions set forth in this document and on the SW1025 contract. General Requirements (define the specific requirements to be met by this project)Mandatory Technical and Business Requirements are defined in Attachment A and Attachment B. Deliverables will bedefined by the project coordinator.Addendum to Terms and Conditions of SW1025 Breach notification. Contractor will disclose any breach of the security ofthe system following discovery or notification of the breach in the security of the data to any resident of Oklahoma whoseunencrypted personal information was, or is reasonably believed to have been acquired by an unauthorized person. Thedisclosure shall be made in the most expedient time possible and without unreasonable delay to DHS Office of InspectorGeneral Information Security Unit.Contractor must sign DHS acceptable use, confidentiality and nondisclosure agreements; user logon authorization; andsubject to background checks.User logon authorization Contractor will not be granted access to DHS information systems without the prior completion and approval of Form06SC004E, Logon Authorization Request for NonDHS Employees. Certain types of access may be required that abackground check and verification be performed prior to granting access. Contractor will notify DHS immediately whenemployees that have access to DHS systems are terminated.
